Coronavirus Epidemic Update 9: Fecal-Oral Transmission, Recovery vs Death Rate

well welcome to another MedCram
coronavirus update and as of today we have 362 deaths which is up from 305
yesterday and we have 17,000 392 infected which is up from 14,000 556 now
this ratio that we’re looking at here is simply the ratio of confirmed deaths to
confirm infected of course people are going to be diagnosed with the infection
before they might have a chance of dying so there’s a time lag there so it’s not
an accurate case fatality or mortality rate but it is something that we can
look at and track the other thing that you might have noticed on the website
that we reference and we’ll make a link to it which is a nice official reporting
dashboard from Johns Hopkins is this 487 recovered we’ll talk more about that as
well what are the criteria for recovered and we’ll talk a little bit about the
Lancet article and also some reporting out of China today so in The Lancet
article which will give you a link to some really good information the
incubation period on average for these 41 patients that were admitted to a
hospital right there in the middle of this epidemic was 5.2 days but 95% of
all of them had the incubation period within 13 days and you know that for
citizens coming to the United States from that area they’re holding them for
at least 14 days to make sure that they do not have the virus incidentally there
was a German plane yesterday that flew in with German nationals and when they
screened them on the plane there was two people that were positive for
coronavirus on that plane so let’s look closer at this Lancet article because I
think it’s very instructive in terms of periods of time and looking at things so
here we have the infection that starts and that incubation pair would go right
in here so on average would be about you know five or six days and then the time
from symptom onset to admission to the hospital that’s a very key thing there
because most patients probably don’t even go to the hospital
and they may do just fine but for those that do go to the hospital it was noted
to be about a seven-day period and then from admission that was a hundred
percent of the cohort that was looked at in the study because they looked at
those 41 patients within a day of admitting to the hospital about 51% of
them developed shortness of breath another day later 27 percent went into a
RDS and if you want more information about a RDS look at our med cram video
on this channel titled how coronavirus kills then another day before they got
to the intensive care unit and they’re about 10 percent or four patients
require mechanical ventilation so what was the outcome of this study and I
think it was very interesting out of the 41 people at the time of publication of
this in The Lancet there were 28 people that were discharged home there were six
people that passed away that’s about 15% and of the 28 that is 68% which leaves
seven that were still alive and in the hospital being treated so the question
is is what were the criteria and this gets back to that number that we see on
the site what was the criteria for allowing these people to go home and
this is very important so from my understanding there are some guidelines
now that are being instituted nationally in China about what constitutes someone
that’s recovered and at least for this hospital it was 10 days without a fever
it was that the chest x-ray was improving because all 41 patients had
abnormal CT scans chest x-rays this next one is important it’s to be discharged
home there had to be no virus detected in the upper respiratory tract and so
that’s a pretty high bar and with that 68% of those people that enter the
hospital were able to go home and deemed cured we don’t know what happened to the
other seven so if we look at just those that entered into the hospital we’re
looking at about a 15% mortality rate in the least
and from there it could go up depending on what those other seven did if one or
two of those died then the mortality rate goes up I stress again this is not
the overall mortality rate of the virus this is the mortality rate for those
people that have to go to the hospital because their symptoms were so severe okay three other stories I want to talk
about real quick there was in the local Chinese media a story that I picked up
and I’ll work in the link to that one as well in the description below a doorknob
was found to have the RNA PCR of the corona virus on it so again this
highlights the need for hand hygiene carrying around alcohol-based hand
sanitizers make sure you’re washing your hands the other thing I want to
highlight is that there was some local media in China about a patient being
discharged home this is a patient that was 30 years old he was admitted on the
20th of January and he was being discharged today on the 3rd of February
again I looked very carefully at the media report of what was the criteria
and they did mention that this was the standard criteria for discharging a
patient home in this case the patient had a normal temperature times 3 days
and they were doing PCR to look specifically for the RNA of this virus
and it was negative and they were checking it at every 24-hour intervals
so this patient felt better the PCR was negative for the virus
wasn’t shedding it in other words and it’s temperature been normal for 3 days
so I think that adds a little bit more in terms of what that number means on
the website people that have recovered think about this though in terms of a
timeline the first thing that’s going to happen is someone’s gonna be a confirmed
case the second thing that could happen is they could die and then the third
thing that would happen is that they would recover obviously they’re not
going to recover after they die but these things are going to happen
in this order so it’s gonna take the longest to have a recovery it’s gonna
take the shortest to have a confirmed case and it’s gonna take intermediate
somewhere between that time to have a death
so look for in the future as we progress through this epidemic that the number of
recoveries is going to surpass the number of deaths and we hope that that
is by a large amount let’s talk a little bit about how it spreads so we all are
familiar with the common cold the flu and the importance for a mask and also
the need for eye protection in this case especially in the healthcare setting
what may be something that we may have underestimated and that is the
fecal-oral route and the reason why that’s the case and we’ll put a link to
this in the description it’s because the man in Washington State they found the
corona virus in his stool and why this is important is because this goes back
to 2002 2003 where there was a big outbreak of SARS in a particular
apartment complex in China and it was because of this toxic viral plume after
the man had diarrhea and that aerosolized the viruses into the air and
it went into the other apartments and that’s where the other people picked up
the virus and so it is possible that this fecal-oral route may be a big
determining factor but it highlights that fecal-oral
transmission of this virus is potentially in play and so washing hands
is an important thing to understand we are witnessing and in the middle of a
human tragedy that is pretty scary to say the least there are a lot of people
that are suffering but there are some things that we can do about this we can
educate ourselves we can understand the virus we can make sure that we’re doing
everything that we can to prevent that virus from spreading we can make a

    Who are checking on the people that's quarantine at home in China every day to make sure that they're not dead. I seen one video eight people in one day and I'm not for sure if it was a whole day or within a few hours in the hospital dead

    First of all this is a great service to the community. Its good that you mentioned that fecal-oral route is a hypothesis. The number of people having gastro track symptoms is very low. The delay between events like diagnosis, death and recovery is really well explained. There is another way to put a good estimate. Severity. Almost 15%-20% diagnosed people are labeled severe. It is possible a 1/2 of them can die (6 out of 13 in the paper you described died, 41 diagnosed). So a number between 5%-10% of diagnosed people may die. But an upper bound of 15-20% and a lower bound of 2% seems to be the most realistic. Your point of diagnosis vs infected is right. The number should be calculated for the diagnosed. And the estimate should be based on diagnosed people. Outside China the death rate is very low (less than 1 percent). There can be three reasons 1) better medical care (low number of patients) 2) A delay 3) virus itself losing its harmfulness when it moves from one host to another (in this case the death rate may actually go down to 1% or lower at the end?)

    We can pray as well. When I was in EMS I almost lost my faith. Around that time I tested God and asked him to prove if he was real. Long story short he did and he did a lot through answering my prayers. Meditation and prayer are an important part of health regardless your spiritual beliefs or lack of. We need to apply that to this outbreak. If you are agnostic or atheist try meditating on healing thoughts for this outbreak or just take a leap of faith and ask the great spirit to work in this situation and in human kind that we evolve past practices that can cause an epidemic like this in the future.

    Good presentation with the numbers being provided, but I'm not buying the numbers provided by China. Looking around SM shows a much more dire situation with the beginnings of a exponential rise in cases- with China having nowhere near the capacity to handle the cases. Plenty of videos of people just keeling over in the public view and I'd imagine a much greater number of those succumbing in their homes.

    Researcher and activist Dane Wigington has been documenting disturbing changes in the environment called geoengineering. These include various weather modification techniques such as chemtrails and HAARP as well as GMO crops.     I don't think "recovered" is the term. I think "got better enough to continue recuperating at home under lockdown" is closer to it. The Chinese are utterly overwhelmed – getting the walking wounded out the door is a priority.

    People in china a getting videos out of the numbers of infected and dead being falsified and are way higher then being reported by the media. There are videos of sick dying and dead lying in the streets. Crematoriums being run 24-7 if this is the case how could 300-400 keep these running so hard unless the figures were lies to keep down panic. other then videos is there anymore confirmation on this?

    If they are recovered, the virus is sleeping. That means the person is still with the virus, it just takes time for the virus to awake again.

    So this procedure is like an eternal now
    So one day, when everyone takes it for-granted as nothing else happened, the virus is still and keeps increasing around the world

    Just hope for recovered medication will be founded, or humanity will end in some years to come

    China coming out today and saying that America has not offered them any aid and has simply cut of travel from China after pulling Americans out of the Epicenter is a total fabrication of the truth, the CDC and everything it has to bring to bear on a situation like this was offered to China and they refused our help. The Chinese government also was arresting doctors who where trying to alert them to the outbreak, one of the first doctors who has since died from the virus was arrested and put in jail. This attempted coverup by the Chinese government does not aid the situation in any way. If they had not tried to cover this up almost 29 Million people would of been kept in China and instead now we have this virus all around the world, this cannot be over looked when international talks resume. There is not a doctor in the US that hasn't said the numbers coming out of China aren't real and that the number of infected is most likely 400,000 plus. Not this 15,000 number they are reporting, the world should demand that China divulge the truth. Why would they not want the CDC on site or the WHO who they own. These outbreaks are killing people all over the world and China owes the world the truth not the version of the truth they want the world to see.

    I don't know if anyone has addressed the literal African Elephant in the room yet. I looked for any WHO report on the impact and spread of this virus in Africa, and found none as of yet. I might also add that China has a strong presence in many African nations, and the last I looked at least 1 million Chinese people live and work in Africa. Not a betting man, but if I were, I would put down good money that this N. coronavirus is already spreading in areas on the African continent. I know that I cannot be the only person noticing this. any ideas?

    Didn't the Lancet article suggest the number of cases was vastly under reported and was possibly as high as 75,000 several days ago? And end Feb infected could be as high as 183 million? 😲

    If the incubation period is 2 roughly 2 weeks, and the total reported number of deaths/recovery is around 849 out of 17,392 confirmed cases, that leaves 16,548 that are still active.

    2 weeks from now are we gonna know roughly what happened to those 16,543?

    I doubt it.

    Fecal oral contamination is huge. When cancer patents take Chemotherapy, they advise to double flush and close the lid and for others to avoid the bathroom due to healthy people being exposed to the chemo drugs for the first 48 hours. Standard low water flush toilets are especially vulnerable I would imagine. And many do not wash their hands after using the bathroom in public stalls.     Question; how does one protect themselves with contact dermatitis? So, I am practicing good hand hygiene, but soap tends to make my skin break out after awhile. So I end up getting really raw hands and eczema with washing my hands too much, even hand sanitizer ends up making me break out. Of course I am still using one or the other method, but yeah.

    It wouldn't be a surprise that a secondary mode of transmission would be fecal-oral particularly if the patient swallows his sputum which is rich in virus. The stomach lining and the gastric juice might not be enough to kill the virus so it could probably made its way through the intestine and rectal route.

    Thank you so much for this video.. I was the person who commented about a 50/50% death rate based on the infected/cured in the last video.. I did not keep in mind that the recovery takes longer + the fact that many people who has the virus might not even seek help since their symptoms are mild.. this is great!

    Most people dont use the toilet correctly in the US. Seriously. Especially for Noro and "puke" bugs. First, in your bathroom, keep "negative pressure" which means leave the exhaust vent on ALL THE TIME. Keep air and viruses moving out out out out of your home. Second and most important, the toilet has a lid for reasons other than slamming in the middle of the night. Vomit or stool, CLOSE THE LID before flushing. As a precaution, keep your families toothbrushes in a medicine cabinet away from settling fecal matter. Lastly, after using the bathroom, wash hands properly and spray in some lysol as you are leaving the room. If you are really sick pretty bad, quarantine yourself inside the bathroom. Grab a pillow and blanket and camp out. Keep it from spreading to your family and back out into the public.

    The time lag between developing symptoms and Death is about 5 to 7 days. So please divide present deaths by the number of cases from five days ago to get a reasonable mortality ratio.

    In my humble view, mobile phones and tablets, or lack of hygiene with devices followed by lack of hand washing, then eating could be a route. The open toilet one is very relevant. Thanks!
